子ども医療費助成制度の現況届提出に関してご案内いたします。
書類を受け取った方は、現況届に保護者の2007年の所得を証明するもの－源泉徴収表・課税証明書等をつけて必ず提出してください。
ただし、加入している健康保険、年金に変更があった人は、保護者および子どもの保険証の写しも添付してください。
提出期限は、2008年8月8日です。
各総合支所くらし応援課保健福祉係または子ども課子ども給付係（区役所２階）まで持参いただくか、郵送で〒105-8511 港区役所子ども課子ども給付係へ。

Did you know that you’re supposed to report annually, the current status of your family’s medical expenses, in order for you to be eligible to receive any kind of subsidies? This is especially important for the reimbursement of your child’s medical expenses.
Once you’ve received the documents regarding your medical expenses, please submit them with a certificate of income for 2007. You might have to include copies of your tax withholding slip, or tax declaration. If you’ve changed health insurance plan, or pension program, please submit copies of your family’s new health insurance card as well.
The deadline to submit these documents is August 8th.
Please bring these documents to the Residents’ Life Support Section, at each Regional City Office, or mail them, to the Children’s Benefits Subsection, the Child Affairs Section, located at the Minato City Office.
