혼자 생활하고계시는 고령자등의 조사를 실시하고 있습니다
시나가와구에서는, 고립사망등을 방지하기 위해서, 구내에 살고 있는 「혼자 생활하시는 고령자」 「고령자만의 세대」등의 긴급 연락처등을 파악하기 위한 조사를 실시하고 있기 때문에, 이해, 협력을 부탁합니다.
기간：８월 ３１일까지
조사 방법：　　　
고령자 상담원이 대상되는 댁을 방문해, 기입 방법을 설명 후, 본인이 「고령자등 연락표」에 기입해, 상담원이 당일 혹은 후일 회수합니다.
대상：거주지의 주위 ５００ 미터 이내에 ２촌수 이내의 친족이 없는 세대로, 다음의 어느 항목에 해당하시는 분
(1)７０세 이상으로 혼자 생활하시는 분
(2)７０세 이상의 분과 ６５세 이상의 분만으로 구성되어 있는 세대
※７０세 이상이란 쇼와 １３년４월１일 이전에 태어나신 분.６５세이상이란 쇼와 １８년４월１일 이전에 태어나신 분.
※고령자 상담원은, 신분 증명서를 휴대하고 있습니다.
※제공해 주신 정보는, 시나가와구 정보 공개·개인 정보 보호 조례에 의거해 적정하게 관리합니다.

Ipinahayang ng lungsod ng Shinagawa na sila ay nagsusurbey ukol sa kalagayan ng mga senior citizens na naninirahan magisa or kasama lamang ang kanilang mga asawa. Naglalayon ang surbey na ito alamin kung may iba pa silang matatatawagan kung mayroong emergency. Hinihingi nila ang inyong kooperasyon.

• Ang panahon ng surbey ay hanggang Agosto 31, 2008.
• Ito ay isang door-to-door surbey. Pupunta ang mga staff sa iyong bahay para sabihin ang detalye ng aktibidad na ito at para ipasagot ang “Emergency Contact Information Card”. Kukunin ng staff ang card na ito sa mga sumusunod na araw.
• Mga Kasapi: Mga senior citizens na walang kamaganak hanggang ikalawang lebel na naninirahan sa loob ng 500 meters. Kasama rin ang mga sumusunod:
a) Una , ang mga lampas sa edad na 70 at naninirahan magisa;
b) Ang mga magasawa na mas matanda sa 65 at 70

Tandaaan na ang mga lampas sa edad na 70 ay mga ipinanganak noon o bago Abril 1, 1938 at ang mga lampas sa edad na 65 ay ang mga ipinanganak noon o matapos Abril 1, 1943.

Kinakailangan na dala ng staff ang kanilang ID sa lahat ng oras.

Lahat ng impormasyon na ibibigay ng mga senior citizens ay proprotektahan ng lungsod ng Shinagawa. Susundan nito ang Public Information Disclousure and Personal Information Protection Regulations na itinayo ng lungsod mismo.

(ENGLISH)Shinagawa City is now conducting a survey, in order to gather the contact details of senior citizens who live alone or with their spouses only, to make sure they have someone to call in case of an emergency. We appreciate your cooperation.

The consultation period will be from now until August 31, 2008.

Survey method:
The City’s senior citizens counseling staff, will be making door-to-door visits to people in the survey, to explain and ask them to fill in the Emergency Contact Information Card. The staff will collect the card that same day if possible, or the following day.

Respondents are citizens who do not have any close relative living within 500 meters of　their homes and those who fit any of the following conditions:
① Those who are over 70 and live on their own
② Households consisting of a couple over 70 and 65 years of age

(* Those over 70 are those who were born on or before April 1st in 1938, and those over 65 are those born on or before April 1st in 1943.
* The city counseling staff are obliged to bring their IDs with them.
* All the personal information provided by the citizens is to be properly managed and protected, abiding by the Public Information Disclosure and Personal Information Protection Regulations established by Shinagawa City.)
